GP Chronic Condition Management Plan
Osteopathy Under the GP Chronic Conditions Management Plan (GPCCMP)
If you have a long-term condition that affects your daily life, you may be eligible for Medicare-subsidised osteopathic treatment through a GP Chronic Conditions Management Plan (GPCCMP). At The Body Project, we’re proud to support eligible patients with tailored, evidence-based care under this program.
What is a GPCCMP?
The GPCCMP is a government-supported initiative (previously known as the Chronic Disease Management Plan or CDM/EPC), designed to help people with chronic or complex conditions access allied health services. It allows eligible patients to receive up to five subsidised osteopathy sessions per calendar year through Medicare, when referred by their GP.
Common conditions that may qualify:
-
Chronic back or neck pain
-
Osteoarthritis or other joint issues
-
Chronic fatigue or fibromyalgia
-
Postural syndromes or repetitive strain injuries
-
Ongoing musculoskeletal or neurological conditions
How it works:
-
Talk to your GP – They will assess your eligibility and prepare a care plan if appropriate.
-
Get a referral – Your GP will send you with a referral for osteopathic treatment under the GPCCMP.
-
Book with us – Bring your referral to your first session and we’ll take care of the Medicare claiming.
-
Integrated care – We’ll communicate with your GP throughout your treatment to ensure continuity and shared goals.
Why choose The Body Project?
With experience treating a wide range of chronic conditions, we focus not only on symptom relief but also on functional improvement, movement education, and long-term self-management strategies. Your sessions will be personalised and built around your care plan.
If you’re unsure whether you’re eligible, feel free to contact us—we’re happy to guide you through the process and collaborate with your GP or care team.
